Need a library card?
Library cards are free for Wisconsin Residents. Fill out a library card application in-person at the library, or use the online form below. Please review the details here to help the process go smoother.
- The form should be used for first-time cardholders only.
- The minimum age for a card is 5.
- *Out-of-state residents are eligible for a fee card.
- IMPORTANT!– When you come to pick up your new card at the circulation desk, please present your driver’s license or state ID with your name and current residential address. If your ID does not have your current address, proof of residence must be shown such as a recent paycheck stub or a utility bill.
Full access library cards are issued in person, and registration forms are available at the front desk or you can use the link below and verify your identification when you come in to pick up your card.

The library will contact you to pickup your new library card.
Online Only Library Card
Only interested in electronic materials?
Sign up for a temporary library card. This will allow you to check out our electronic materials such as ebooks, e-audiobooks, and digital magazines.
You cannot check out physical materials with an e-card. If at any time you wish to upgrade your e-card to a library card, simply fill out the library card registration form.
